Strategy[]
Mileena's passive works almost similar to characters that give 1 bar of power at the start of the match (e.g. Kung Jin/Shaolin & Kenshi/Balanced). It will give her 1 free bar of power if she is suffering from bleed. The effects are additive as even though bleed cannot be stacked, it can still be reapplied, which can result in yet another bar of power even though she is already suffering from bleed. As any attack can heal Mileena so long as the enemy is bleeding, both basic and special attacks can be used. The life drain part of her passive can be enhanced with the talent "Mileena's Hunger" which can increase special damage against enemies suffering from bleed, healing her by a larger amount. The Razor Sai is perfect for her as it increases the damage dealt against bleeding enemies which can heal her for even more health as well as providing a static life drain on Special 2 which can still trigger even if the enemy isn't bleeding. The life drain from her equipment and passive can stack.

Please note that battles are highly situational: this section only mentions cards that have mechanics that, for any reason, are particularly effective with/against, or particularly ineffective against Mileena. Having a card that "counters" Mileena does not mean it is always a good choice.
Good with[]
- Any characters that can inflict Bleed.
- Outworld supports such as Kotal Kahn/Sun God, Reptile/Nimble and Kitana/Mournful.
- Mileena/Klassic: Klassic Mileena heals female teammates if she defeats an opponent, and can inflict Bleed.
- Bladed Whips & Dragon Fang equipments: These equipments have a chance to apply Bleed at the start of the match.
Good against[]
- Like how she is good with any character that can inflict bleed, she is also good against them. Especially Skarlet.
- Baraka/Scourge: Getting hit by his improved combo ender would result in 2 bars of power for Mileena.
- Mileena/Vampiress: Her bleed every 10 seconds against Piercing Mileena could easily get Piercing Mileena to Special 2 or X-Ray.
- Kitana/Assassin and Mileena/Ravenous: Their Tag attacks cause bleed which benefits Piercing Mileena, as she could easily retaliate against both of these characters. Ravenous Mileena's passive also grants Piercing Mileena +80-100% more damage on Bleed DOT, giving her a great upper hand and heal her even more on any attacks thanks to her own passive.
- Scorpion/Hanzo Hasashi: Due to Scorpion‘s passive to apply Bleed DOT on Combo Ender and Basic Attacks, every time Scorpion applies Bleed DOT on Mileena she will gain a bar of power.

- D'Vorah/Standard and Kitana/Day of the Dead: These characters are immune to Bleed.
- Jade/MK11 and Raiden/Dark: Both these characters heal from Mileena's Bleed, with Jade spreading this benefit with her MK11 teammates.
- Kitana/Ronin, Cassie Cage/Undercover, Katana and Champion's Nunchaku equipment: All of these inflict more damage against Outworld characters.
- Liu Kang/Klassic: Liu Kang gains a critical chance boost from Mileena's bleed.
- Skarlet: Skarlet can heal from Mileena's Bleed with her Vial of Infinite Blood equipment.
- Piercing Mileena herself. SP2 will give her an extra bar of power, so it's recommended to avoid using it against her.
