Please note that battles are highly situational: this section only mentions cards that have mechanics that, for any reason, are particularly effective with/against, or particularly ineffective against Mileena. Having a card that "counters" Mileena does not mean it is always a good choice.
Good with[]
- Any character that can cause bleed. Not only will Mileena be able to gain REGEN from SP2, but Netherrealm characters gain boosted damage against bleeding enemies.
- Mileena/Ravenous, Mileena/Piercing: Ravenous Mileena's passive not only gives the opponent bleed on tag-in to allow Vampiress Mileena a REGEN opportunity on SP2, but she also grants the whole team +80-100% damage boost on Bleed DOT. Piercing Mileena can gain REGEN on attacks thanks to Vampiress Mileena's passive.
- Kitana/Assassin: her tag-in attack applied Bleed DOT to the opponent, giving Mileena yet another opportunity to gain REGEN on SP2.
- Scorpion/Klassic: an amazing teammate for Mileena, as he is the only Scorpion capable of applying bleed with his SP1. Furthermore, since Scorpion can pull in the most wounded enemy from the enemy team with his SP1, tagging in Mileena to use her grapple SP2 will often result in a kill while granting her REGEN. Plus, as a Netherrealm character, Scorpion also benefits from the damage boost against bleeding enemies granted by Mileena's passive.

- Opponents that are affected by bleed. Mileena's passive grants Netherrealm characters +50% boosted damage on attack.

- Raiden/Dark: instead of taking damage, Raiden heals from the Bleed DOT Mileena applies when active.
- Jade/MK11: Jade's passive allows herself and other MK11 characters to heal from bleed.
- Liu Kang/Klassic: Mileena's Bleed gives Liu Kang extra Critical Attack Boosts.
- Mileena/Piercing: Piercing Mileena gains +1 bar of power from Vampiress Mileena's bleed.
- Tremor/Aftershock: Tremor and his fellow Martial Artist teammates gain unblockable chance against Netherrealm characters.
